<h3>Job Description</h3><p>Job Description<p><strong>Nurse Practitioner – Youth Wellness Hub (Part-Time, Permanent)</strong><br /><strong> </strong><br /><strong>Location:</strong> Scarborough, ON (Hybrid work model)<br /><strong>Hours:</strong> 21 hours per week<br /><strong>Compensation:</strong> $48.35–$68.48/hour<br /><strong>Employment Type:</strong> Permanent, Part-Time<br><br><strong>About the Role</strong><br /> We’re seeking a dynamic and compassionate <strong>Nurse Practitioner</strong> to support youth aged 12–25 within an integrated health and wellness hub. This role offers a unique opportunity to provide rapid, barrier-free access to primary care for young people navigating mental health, substance use, and wellness challenges.<br><br><strong>Key Responsibilities</strong><br /><ul><li>Deliver comprehensive episodic primary care for youth and young adults.</li><li>Perform full assessments, including health history and psychosocial factors.</li><li>Diagnose and manage acute and stable chronic conditions.</li><li>Prescribe medications and order diagnostic tests within NP scope.</li><li>Provide health education and integrate psychotherapeutic approaches as needed.</li><li>Collaborate with a multidisciplinary team (counsellors, peer workers, navigators).</li><li>Participate in program development, quality improvement, and community outreach.</li><li>Provide mentorship and contribute to knowledge-sharing within the team.</li></ul><br /><strong>Qualifications</strong><br /><ul><li>Master’s degree in Nursing (Primary Health Care NP specialization).</li><li>Registration in good standing with the <strong>College of Nurses of Ontario (Extended Class)</strong>.</li><li>Experience working with youth and/or in mental health, addictions, or primary care.</li><li>Knowledge of trauma-informed, harm reduction, and anti-oppressive practices.</li><li>Strong decision-making, communication, and leadership skills.</li><li>Ability to work hybrid (on-site and virtual) and flexible hours, including evenings.</li></ul><br /><strong>What We Offer</strong><br /><ul><li>Competitive compensation package.</li><li>Flexible scheduling and hybrid work model.</li><li>Opportunity to make a tangible impact on youth health and access to care.</li></ul><br /><strong>Interested?
