You have clinical skills and extensive knowledge of wound, ostomy and continence care management for a wide range of patients with chronic diseases.</p><ul><li>Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns.</li><li>Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of our team</li><li>Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes</li><li>Continually assess, anticipate, identify and respond to new and changing needs of our patients</li><li>Travel between patients’ homes within a specific geographical region</li></ul>Qualifications<br><br><ul><li>Must have completed NSWOC or International Interprofessional Wound Care Course (IIWCC)</li><li>Currently registered and in good standing with the College of Nurses of Ontario (CNO)</li><li>Minimum of two-year nursing experience (Community Nursing experience is ideal, but not required)</li><li>Ability to work independently in managing your own caseload</li><li>Proficiency in English, both verbal and written required (if you can speak additional languages, terrific)</li><li>Valid Ontario driver's license and reliable vehicle with willingness to travel</li><li>Basic proficiency in Word, Excel and ability to learn software</li></ul><p>#Peel_nurse</p><br><br>Additional Information<br><br><p></p><p><strong>Spectrum Health Care </strong>is thrilled to be named one of <strong>Canada’s Best Managed Companies.</strong> This award recognizes top companies across Canada for overall performance, sustained growth, strategy, capabilities and innovation, culture and commitment, and leadership.
