<h3>Job Description</h3><p>Job Description<p><p> </p><p>Registered Nurse/Registered Practical Nurse (Contract)</p><p><strong>Position Overview</strong></p><p>As a <strong>Registered Nurse (RN)</strong> or <strong>Registered Practical Nurse (RPN)</strong>, you are passionate about empowering clients and their families to maintain independence through health education and hands-on care. Working collaboratively with the <strong>Care Designer</strong> and the interdisciplinary care team, you will help create a safe, innovative, and client-centered environment that supports each person’s unique goals and needs.</p><p><hr /></p><p><strong>Key Responsibilities</strong></p><ul><li>Conduct comprehensive health assessments for clients and their family members or caregivers.</li><li>Identify care needs and develop personalized, goal-oriented care plans.</li><li>Provide health teaching and hands-on training to clients and families on daily living activities such as:</li><ul><li>Personal care</li><li>Medication management</li><li>Mobility and safety</li></ul><li>Support clients and families in navigating the healthcare system and connecting with community resources.</li><li>Monitor and evaluate client progress, making adjustments to care plans as needed to ensure best practices and safety.</li><li>Maintain accurate and timely documentation of assessments, teaching, and care plans.</li><li>Communicate effectively with clients, families, and the healthcare team to ensure integrated, coordinated care.</li><li>Promote safe, client- and family-centered care consistent with each individual’s home environment and life circumstances.</li><li>Utilize mobile and electronic applications for documentation and communication (e.g., phone, email, AlayaCare).</li></ul><p><hr /></p><p><strong>Availability</strong></p><ul><li><strong>Status:</strong> Casual, Contract Position</li><li><strong>Flexibility:</strong> Required for scheduling and client visits</li></ul><p><hr /></p><p><strong>Reporting & Relationships</strong></p><ul><li><strong>Reports To:</strong> Care Designer</li><li><strong>Collaborates With:</strong> Care Designers and interdisciplinary care team</li></ul><p><hr /></p><p><strong>Key Performance Indicators</strong></p><ul><li>Attendance and reliability</li><li>Compliance with AMS (Attendance Monitoring System – AlayaCare)</li><li>Quality of client care and documentation</li></ul><p><hr /></p><p><strong>Qualifications & Requirements</strong></p><ul><li>Current registration in good standing with the <strong>College of Nurses of Ontario (CNO)</strong> as an RN or RPN</li><li>Minimum <strong>3–5 years of nursing experience</strong> in a clinical or community health setting (home care experience is an asset)</li><li>Demonstrated excellence in health teaching and communication with a compassionate, client-centered approach</li><li>Two positive professional references</li><li>Valid <strong>CPR and First Aid Certification</strong></li><li><strong>Valid driver’s license</strong> and access to a reliable vehicle</li><li>Ability to travel independently throughout Ontario</li><li><strong>Criminal record check</strong> (vulnerable sector clearance)</li><li>Strong written and verbal communication skills in English</li><li>Proficiency with mobile applications and electronic documentation systems</li></ul><p> </p></p></p>Create Your Resume First
